Media-reported artificial intelligence incidents involving content generation reached nearly 500 by January 2026, according to the OECD AI Incidents and Hazard Monitor. The count stood near 50 in early 2020 and passed 200 in early 2024, which places most of the growth inside a two year window.

The AI Incident Database recorded 362 incidents during 2025, up from 233 in 2024. Cumulative entries reached roughly 800 to 900 unique incidents through the first quarter of 2026, with new logging running at an estimated 130 to 180 incidents per year and year over year growth in the range of 35 to 45 percent.

Category distribution has shifted with the technology. Misinformation and content harms account for approximately 28 percent of database entries, discrimination and bias roughly 22 percent, and physical safety failures about 14 percent. Generative systems produced approximately 58 percent of 2025 entries, reversing the earlier pattern in which recommendation engines and computer vision systems generated most logged incidents.

Reported incident growth has outpaced measured AI deployment growth. That gap suggests improving detection and reporting rather than a proportional rise in failure rate, though the databases do not separate the two effects.
