The National Highway Traffic Safety Administration opened a Special Crash Investigation after a Tesla Model 3 crashed into a house near Houston, Texas, on June 19, 2026, killing a 76-year-old woman inside, according to Al Jazeera's report of June 23.
The role of Tesla's Full Self-Driving system in the crash is disputed. The sheriff's office said the driver reported using the automated driving assistance system. Tesla's vice president of AI said the driver manually overrode the system and held the accelerator down throughout, reaching 73 mph. Tesla CEO Elon Musk wrote that Full Self-Driving drives slowly through neighborhood streets and that this was a high-speed crash. Investigators said the driver showed no signs of intoxication and cooperated with the inquiry.
NHTSA announced the Special Crash Investigation itself. The agency has not published findings on whether the driver-assistance software was active at impact.
Missy Cummings, director of the autonomy center at George Mason University, said Tesla's camera-only approach means the system does not perform well as a driver-assist feature either. Tesla maintains that its self-driving technology poses no safety hazard and says it is up to 10 times safer than human drivers.
The crash is categorized as a physical-harm incident because a death occurred while a Tesla driver-assistance feature was reported to be in use.
