Reported autonomous vehicle incidents in the United States reached 5,202 as of November 17, 2025, according to federal crash data compiled by industry analysts. Across all reported autonomous vehicle events, the data recorded 451 injuries and fatalities, including 65 fatalities.

The severity rate remains a closely watched figure. About 7.4 percent of autonomous vehicle incidents resulted in injury and 1.2 percent resulted in a fatality. Monthly totals hit a record in early 2026, with 118 reported crashes in January, the highest monthly figure since data collection began in July 2021, a rise that tracks with expanding deployments.

The data separates two technology categories. Systems that assist a human driver, known as ADAS, and fully automated driving systems, known as ADS, are reported differently. Tesla accounted for the most ADAS-related incidents at 2,093, reflecting its large deployed fleet, while Waymo reported the most fully automated system crashes at 907.

Analysts caution that raw incident counts rise as more vehicles operate on public roads, so context matters when comparing companies or years. The figures nonetheless underscore how autonomous driving safety has become a central data point for regulators, insurers, and the public as robotaxi and driver-assist systems scale across US cities.
